STAFF NURSE

Position: Staff Nurse

• Provide initial point of contact for patients
• Solicit pertinent clinical information utilizing assessment skills during the intake process to identify and determine the urgency of the requested referral and appropriate care team needs.
• Serve as a clinical expert to assess and recommend appropriate resources.
• Utilize triage skills to determine if further testing is required and implement the appropriate ordering process.
• Ensure all necessary medical records are updated
• Exhibit sound nursing judgment in coordinating multiple, complex case management scenarios for patients requiring care and services.
• Work collaboratively with onshore team members to meet the patient’s needs.
• Assist members with locating and connecting with community resources
• Handle and resolve care coordination escalations received from other departments and vendors
• Collaborate with other care team members such as primary care providers, specialists, pharmacists, case managers, etc.
• Prepare, complete, and maintain clinical documentation in clinical systems through written case notes and contacts and perform case follow-ups timely and in accordance with the Model of Care
• Deliver administrative support to various department(s)
• Perform any other job-related instructions, as requested
