1.) Documentary Requirements:
A. Valid, Available and On-Hand, Philippine Passport of A MINIMUM VALIDITY OF EIGHTEEN (18) MONTHS. “cannot not accept Applicants with lost or damaged passports or pending for release or withdrawal from another party such as DFA or another Agency etc..
B. Valid NBI Clearance.
C. Updated resume.
2.) Qualification Requirements:
A. Professional Regulatory Commission (PRC) License of A MINIMUM VALIDITY OF EIGHTEEN (18) MONTHS.
B. Professional Regulatory Commission (PRC) Certificate.
C. Professional Regulatory Commission (PRC) Board Rating Certificate.
D. Transcript of Records (TOR) & Diploma of a Bachelor Degree in PHYSICAL THERAPY.
E. Except for Fresh Graduates, all applicants must present Certificate/s of Employment of a minimum of ONE (1) year of “continuous” Employment, clearly indicating the Applicant’s Job Title as PHYSICAL THERAPIST.
