A. JOB SUMMARY
To provide accurate and timely dispensing of right medications to the right patient (in-patient & out-patient) with adherence to the organization’s set standards, policies, medication safety and government rules & regulations.
B. STATEMENT OF DUTIES AND RESPONSIBILITIES
TECHNICAL:
1.Prepares/issues medication requests (e.g. IVS, oral, external preparations) for in-patients or out-patients.
2.Prepares/Issues Dangerous Drugs/Controlled substances requested by Nursing Units for Floor Stock use as well as for individual patient’s use.
3.Refers unavailable/non-formulary drugs with available generic or therapeutic equivalents to the physician through the nurses.
4.Receives and checks medications for return/turn-in from nursing units or other department.
5.Reviews/Approves floor stock requests based on Master List for filling by Pharmacy Assistant.
6.Oversees the preparation of non-sterile preparations, extemporaneous compounding, galenicals, etc. performed by Pharmacy Assistant.
7.Instructs Nurses, Nursing Aides on the proper use, storage and handling of drugs.
8.Provides drug information as necessary to pharmacy, medical, paramedical and nursing staff.
9.Checks requests for special purchase order of non-formulary and out of stock drugs prior to sourcing and order placement by Pharmacy Assistant.
10.Ensures adequate control and documentation in the dispensing of controlled substances.
11.Provides adequate drug control ensuring that drugs are properly handled and stored especially controlled substances, SALAD, HAM, hazardous substances and cold storage products.
12.Receives endorsement from outgoing shift.
13.Oversees the work of the Pharmacy Assistant.
14.Maintains professional competence particularly in knowledge of drug stability and compatibility.
15.Conducts cycle count and annual physical count of drugs and pharmaceuticals.
16.Submits pertinent reports required by the department head.
17.Complete all competence/skills assessment requirements.
18.Ensures compliance with all applicable government rules and regulations.
19.Answer queries related to the services of the department.
20.Ensures proper usage and maintenance of equipment and facilities.
21.Maintains strict confidentiality of patients and employee information.
22.Attends seminars/workshop and other staff development programs relevant to the work.
23.Attends pharmacy staff meetings and prepares minutes of meeting as assigned.
24.Keeps the dispensing and compounding area neat, clean and orderly.
25.Performs other tasks that may be assigned.
C. BASIC RELATIONSHIP
Regular coordination with the Staff, Department Manager, Purchasing, Nursing, doctors and other paramedical staff for regular activities.
D. DIRECT REPORTS
Reports to the Department Manager for daily activities and solicitation of directions, guidance in the performance of assigned tasks.
E. QUALIFICATION
Academic/Professional Qualifications:Graduate of B. S. Pharmacy
Above 75% board Rating
Duly registered/Licensed
Relevant Experience:Preferably atleast one year experience in hospital
setting, but Fresh Graduates may apply
