Job Description

IN-PATIENT PHARMACY DISPENSING PERSONNEL
1. Fills and dispenses all requisitions form different nursing care units and special areas.
2. Checks display on shelves and compose requisitions of medicines from the Pharmacy Stockroom.
3. Answers telephone inquiries.
4. Responsible for the cleanliness of the shelves assigned to her/him and monitors the expiry dates of drugs in those shelves in accordance with the pull-out policy.
5. Does physical inventory in the assigned shelves every month. All expiry items will be withdrawn on the 1st day of the month for sealed items and loose medicines.
6. Responsible in making paper tabs and other non-sterile preparations in special areas for in-patient use.
7. Responsible for charging medications and supplies to the patient’s account.
8. Posts charged items from the patient’s requisition form.
9. Logs room temperature and bioref temperature every shift.
10. Attends regular unit meetings, and be involved in seminars and trainings pertinent to the profession or self-augmentation.
11. Performs other functions and responsibilities which may be assigned by the immediate superior.

RECEIVING PHARMACIST
1. Receives physician’s drug order sheet faxed from the nurse station and checks for erroneous, violative and impossible physician’s order.
2. Clarifies any vague physician’s orders by informing the station’s charge nurse.
3. Record and file those orders designating one medication profile for each patient.
4. Coordinates with resident physicians, charge nurses and staff nurses for any inquiries or problems that may arise.
5. Verifies medication profile/ order versus the medications requested by the nurses before releasing the drugs.
6. Makes sticker tickets and charge the patient with materials to be used in compounding the medication.
7. Answers telephone drug inquiries.
8. Responsible in disinfecting and cleaning of the clean bench before, during and after compounding.
9. Compounds narcotic IV drugs, non-sterile preparations- KCl solution, KMnO4 for wound, IV admixtures, TPN (if specified by the physician).
10. Updates and checks census every night shift and pulls out medication profile of discharged patients.
11. Attends regular unit meetings, and be involved in seminars and trainings pertinent to the profession or self-augmentation.
12. Performs other functions and responsibilities which may be assigned by the immediate superior.

UNIT DOSE PHARMACIST
1. Unit Dose Pharmacist will print Patient’s Unit Dose Report as his/her reference on charting prior to charging.
2. Unit Dose Report shall be updated against patient’s Medication Administration Guide as he/she goes on rounds for patient chart review on each in-patient under charge account.
3. Records all standing oral medication orders, its dose, dosage form, duration, and timing of each drug.
4. Takes note and pulls out drugs from patient’s drug cubicle that were discontinued, held, shifted, and revised then issues a credit memo form for the returned medications.
5. Counts the remaining drugs and charges medications good for 24 hours.
6. Pack medications per patient per room and delivers to each nursing care unit, except special areas.
7. Attends regular unit meetings, and be involved in seminars and trainings pertinent to the profession or self augmentation.
8. Performs other functions and responsibilities which may be assigned by the immediate superior.

OUT-PATIENT PHARMACY DISPENSING PERSONNEL
1. Fills and dispenses prescription and non-prescription medications/drugs for in patient and out-patient.
2. Receives stocks and signs the requisition forms from the stockroom clerk.
3. Displays all medicines on the shelves taken from the warehouse, and is responsible for the cleanliness of the shelves assigned to her/him and monitors the expiry dates of drugs/supplies according to the pull out policy.
4. Carries out the responsibility endorsed by the Pharmacist of the previous shift.
5. Answers all telephone inquiries.
6. May be assigned as checker or cashier with the same duties and responsibilities of a Checker or Cashier.
7. Attends regular unit meetings, and be involved in seminars and trainings pertinent to the profession or self-augmentation.
8. Performs other functions and responsibilities which may be assigned by the immediate superior.

Qualifications/Requirements

Qualifications

  • A graduate of Bachelor of Science in Pharmacy
  • With or without experience
  • With active PRC License
  • Willing to start as soon as possible
