Qualification Standards
Education : Bachelor’s degree major in Nutrition, Dietetics or
Community Nutrition
Training : Four (4) hours of relevant training
Experience : One (1) year of relevant experience
Interested and qualified applicants are advised to submit the following requirements, fastened in a legal-size white folder (8.5 x 13):
1. Application letter addressed to:

2. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) and CSC Form No. 212 Attachment – Work Experience Sheet which can be downloaded at www.csc.gov.ph;
3. PRC License or Certificate of Eligibility (if applicable);
4. Certified True Copy of Diploma / Certificate of Graduation and Transcript of Records (TOR);
5. Photocopy of PRC Board Rating (if applicable);
6. Certificate of Employment (if applicable);
7. Individual Performance Commitment Review (IPCR) in the last rating period (if applicable);
8. Certificate of Duties and Responsibilities (if applicable) or;
9. Certificate of Trainings for each training indicated in the PDS
