INSTRUCTIONS ON HOW TO APPLY:
1. ZCMC shall be accepting online applications from September 23 – October 4, 2024 only.
2. Interested and qualified applicants must register online at the ZCMC Applicants’ Registry using the link below:

3. Application letter stating the position applied must be addressed to:

4. Applicants are required to scan the application documents listed below. Said documents should be scanned and uploaded in one file PDF format at the ZCMC Job Portal. Make sure the PDS is updated, signed and with the recent picture attached to it. The PDS and Work Experience templates can be downloaded at www.csc.gov.ph.
Reminders before submitting application:
1. Please use the PERSONAL DATA SHEET (CSC Form No. 212, REVISED 2017) and fill it out with your information CORRECTLY.
a. under Part V. WORK EXPERIENCE
Include private employment. START FROM YOUR RECENT WORK
b. under Part VII. LEARNING AND DEVELOPMENT
START FROM THE MOST RECENT AND RELEVANT L&D
c. the PDS must bear the applicant's signature and date of accomplishment at the bottom of every page.
d. all information should be provided accurately.
e. do not leave blank entries. Put N/A if not applicable.
f. do not forget to indicate your response to questions 34 to 40 on the right side of the sheet.
g. provide details or specifications for any “Yes” response.
h. as agreement to and for completion of the PDS, the applicant’s signature and right thumb mark should be affixed in the boxes provided.
i. picture must be taken within the last six (6) months.
2. Attach documents as mentioned in your PDS and see the list of requirements to be submitted.
3. SCAN YOUR APPLICATION/S CLEARLY INTO ONE (1) PDF FILE ONLY.
INCOMPLETE ATTACHMENT WILL NOT BE PROCESSED.
Following are the documents to be submitted:
1. Application Letter specifying the position applied for.
2. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture
(CS Form No. 212, Revised 2017)
3. Work Experience Sheet (CS Form 212, Rev. 2017)
4. Original/Authenticated certificate of eligibility/rating and updated PRC license
5. Original/Authenticated Diploma
6. Original/Authenticated TOR (for employment purpose only)
7. Certificate of Residency and/or Fellowship Training
8. Diplomate and/or Fellow Certificate
9. Performance Rating in the last rating period with at least Very Satisfactory Rating (for promotion)
10. Certificate/s of relevant trainings attended
11. Certificate/s of relevant experience
12. Photocopy of updated PHIC Accredited ID
