1. For DOH-Central Office applicants applying for *permanent/regular position, please submit your application addressed to the Director IV, Health Human Resource Development Bureau, DOH, Manila.
2. For DOH-Central Office applicants for *job order/contract of service position, please submit your application addressed to the Head of Office/Bureau where the vacancy is.
3. For *FDA, BOQ, TRCs and DOH Hospitals applicants, please submit your Application Letter Addressed to the Head of Office where the vacancy is.
4. Four(4) copies of duly accomplished Personal Data Sheet together with the Work Experience Sheet (CSC Form No. 212 Revised 2017)/Resume (indicating education,
work experience, no of hrs. of trainings/seminars attended, eligibility).
5. Authenticated copy of Diploma and Transcript of Records(for new entrants/applicants).
6. Copy of Certificate of Trainings/Seminars attended.
7. Copy of Eligibility (Career Service Sub-Professional/Professional; Board Rating; PRC ID license).
8. Two(2) copies of performance rating for the last two rating periods. (For applicant applying for promotion in a regular/permanent position).
9. Certificate of employment with statement of duties and responsibilities of previous/current employment.
