In-Patient Pharmacy Dispensing Personnel

< !--[if !supportLists]-->2     <!--[endif]-->Checks displays on the shelves and composes requisitions of medicines from the main pharmacy

< !--[if !supportLists]-->3     <!--[endif]-->Answers telephone inquiries

< !--[if !supportLists]-->4     <!--[endif]-->Responsible for the cleanliness of the shelves assigned to her/him and monitors the expiry dates of drugs in those shelves in accordance with the pull out policy

< !--[if !supportLists]-->5     <!--[endif]-->Does physical inventory in the assigned shelves every first week of the month;All expiry items will be withdrawn on the 1st day of the month for sealed items and at the end of the month of expiry for loose medicines.

< !--[if !supportLists]-->6     <!--[endif]-->Receives telephone manual posting of medication from OR mini-pharmacy; sends a reprint of charges to OR information desk nurse and secretary for out-patients under cash basis accounts

< !--[if !supportLists]-->7     <!--[endif]-->May be assigned in the Pharmacy Office with the following duties and responsibilities:

< !--[if !supportLists]-->a     <!--[endif]-->Purchases non-formulary drugs

< !--[if !supportLists]-->b    <!--[endif]-->Prepares printed out-patients requests from Korean and Japanese Help Desk HMO and CDUH stock holders in accordance to the guidelines

< !--[if !supportLists]-->c     <!--[endif]-->Prepares all supply requisitions from the different departments found in Cebu Doctors’ University Hospital and from the different sister companies of Cebu Doctors’ University Hospital (NGH, SGH, MDH, ODH)

< !--[if !supportLists]-->8     <!--[endif]-->Attends regular departmental meetings, and be involved in seminars and trainings pertinent to the profession or self augmentation

< !--[if !supportLists]-->9     <!--[endif]-->Performs other functions and responsibilities which may be assigned by the immediate superior

 

RECEIVING PHARMACIST

< !--[if !supportLists]-->1     <!--[endif]-->Receives physician’s drug order sheet faxed from the nurse station and checks for any erroneous, violative, and impossible physician’s order

< !--[if !supportLists]-->2     <!--[endif]-->Clarifies any vague physician orders by informing the station’s charge nurse

< !--[if !supportLists]-->3     <!--[endif]-->Records and files those orders designating one medication profile for each patient

< !--[if !supportLists]-->4     <!--[endif]-->Coordinates with resident physicians, charge nurses and staff nurses for any inquiries or problems that may arise

< !--[if !supportLists]-->5     <!--[endif]-->Verifies medication profile versus the medications requested by the nurses before releasing the drugs

< !--[if !supportLists]-->6     <!--[endif]-->Does profile review of patients for any Large Volume Parenteral medication that will due within their shift

< !--[if !supportLists]-->7     <!--[endif]-->Makes sticker tickets and charges the patient with materials to be used in compounding the medication

< !--[if !supportLists]-->8     <!--[endif]-->Answers telephone drug inquiries

< !--[if !supportLists]-->9     <!--[endif]-->May be assigned in the Pharmacy Office when the need arises

< !--[if !supportLists]-->1 . <!--[endif]-->Attends regular departmental meetings, and be involved in seminars and trainings pertinent to the profession or self augmentation

COMPOUNDING PHARMACIST

< !--[if !supportLists]-->1     <!--[endif]-->Orders from the Receiving Pharmacist, counter-checks the Physician’s Drug Order Sheet, and creates a Medication Profile for each patient’s Large Volume Parenterals (Incorporation/Infusions) and Controlled Drugs

< !--[if !supportLists]-->2     <!--[endif]-->Observes proper Personal Protective Equipment (PPE) before compounding

< !--[if !supportLists]-->3     <!--[endif]-->Compounds controlled intravenous drugs and medication that needs to be incorporated or infused in a large volume parenteral preparation under aseptic condition

< !--[if !supportLists]-->4     <!--[endif]-->Attaches sticker tickets and all necessary auxiliary labels to the prepared medication affixing his signature on the drug label and sealed for dispensing

< !--[if !supportLists]-->5     <!--[endif]-->Disinfects the clean bench with 70% alcohol and UV light 15-30 minutes every end of the shift

< !--[if !supportLists]-->6     <!--[endif]-->Disinfects the clean bench with Solarsept Solution scheduled every other Saturday for total eradication of potential disease causing microorganism

< !--[if !supportLists]-->7     <!--[endif]-->Responsible in maintaining the cleanliness of his working area and the availability of the materials used in compounding medications

< !--[if !supportLists]-->1 . <!--[endif]-->May be assigned in the Pharmacy Office when the need arises

< !--[if !supportLists]-->8     <!--[endif]-->Attends regular departmental meetings, and be involved in seminars and trainings pertinent to the profession or self augmentation

UNIT DOSE PHARMACIST

< !--[if !supportLists]-->1     <!--[endif]-->Prints Patient’s Unit Dose Report as his/her reference on charting prior to charging

< !--[if !supportLists]-->2     <!--[endif]-->Shall be updated against patient’s Medication Administration Guide as he/she goes on rounds for patient chart review on each in-patient under charge account

< !--[if !supportLists]-->3     <!--[endif]-->Records all standing oral medication orders, its dose, dosage form, duration, and timing of each drug

< !--[if !supportLists]-->4     <!--[endif]-->Takes note and pulls out drugs from patient’s drug cubicle that were discontinued, held, shifted, and revised then issues a credit memo form for the returned medications

< !--[if !supportLists]-->5     <!--[endif]-->Counts the remaining drugs and charges medications good for 24 hours

< !--[if !supportLists]-->6     <!--[endif]-->Packs medications per patient per room and delivers to each nursing care unit, except special areas

< !--[if !supportLists]-->7     <!--[endif]-->May be assigned at the pharmacy office when the need arises

< !--[if !supportLists]-->8     <!--[endif]-->Attends regular departmental meetings, and be involved in seminars and trainings pertinent to the profession or self augmentation
