c.) Photocopy of Authenticated Certificate of CS Eligibility or PRC Board Rating and updated
PRC License;
e.) Photocopy of authenticated Transcript of Records and Diploma;
f.) Photocopy of certificate/s of Training /Seminars/ Lectures attended;
g.) Photocopy of certificate/s of relevant experience (with Job Description); and
