c.) Performance rating in the present position for one (1) rating period (for those currently connected in the gov’t.
. service)
d.) Authenticated certificate of PRC Board Rating and updated PRC License
e.) Photocopy of Residency Training Certificate;
f.) Photocopy of PhilHealth Accreditation Number;
g.) Photocopy of authenticated Transcript of Records and Diploma;
h.) Photocopy of certificate/s of Training /Seminars/ Lectures attended;
i.) Photocopy of certificate/s of relevant experience (with Job Description)
